AUTHORIZATION FOR PRESCRIBED AND OVER THE COUNTER MEDICATION ADMINISTRATION AT SCHOOLS

If your child will require any medication while at school for the 2021-2022 school year, please have your child’s doctor fill out one of the attached forms for each medication that will be needed. Once completed, drop off the form with the medication to the school health office after July 26, 2021.
